Instructions/Rules:

- Fields marked with an * are required.
- Once an issue has been resolved, change its Status to "Resolved". The notes & issue will be moved to the "History" section of the Issues page. All notes & issues are archived in that page.
- To revert a issue back, change the status to anything but "Resolved".
